Explanation:
YO creo que debes diferenciar los **datos/ficha del paciente"" como pueden ser la dirección, el teléfono, número de SS, etc. del Medical Record, que equivale a nuestra historia clínica.
Esto que copio a continuación sobre la historia clínica se discutió justamente en otro foro:
Dentro de la historia clínica por lo general figura la "Past history"
(Antecedentes personales), "Family history" (Antecedentes familiares), a veces "Social history" (Antecedentes sociales [o socioeconómicos]) y se agregan los datos que surgen del examen físico, estudios de laboratorio,
estudios de imágenes, etc.
Esto lo hace el médico, eso que tu preguntas me da la impresión que lo llena una enfermera o alguien en admisiones. Una tocaya.
