Te doy otra hint de algo que veo en tu lista: Primary Care Physician. Tiene ya una traducción "oficial" ampliamente aceptada en USA, por lo menos: Médico primario o principal (o simplemente "el PCP", como le llaman informalment los hispánicos del norte).
