Leyendo un poco más sobre este departamento, creo que ofrece asistencia/intervención social, por ejemplo:
- reintegración/reinserción social para menores que han cometido actos delictivos.
- asistencia posthospitalaria, para aquellos que han sido dados de alta tras permanecer internados en una institución.
Es difícil saber a qué ámbito específico se refiere "aftercare planning", lo que está claro es que es una "PLANIFICACIÓN DEL SEGUIMIENTO O DE LAS MEDIDAS DE INTERVENCIÓN POSTERIORES".

Aftercare planning can include step down services to a less restrictive environment such as a group home, a therapeutic foster home, intensive outpatient treatment, drug relapse prevention program, Life Skills Training, or inhome services such as IICAPS dependent on the discharge plan for the child. Said aftercare services primarily address the child's ongoing needs for his/her problem, and can include as well reintegration into the community, and reunification services if applicable.
