Mircea was striding towards us, and I was not up to a verbal fencing match with him tonight. Hell, I lost those even when my brain didn't feel like it was about to throb out of my skull. "Hold on" I said, rally hoping I could manage one more shift before I passed out.

Mircea estaba caminando a grandes pasos hacia nosotras y yo no estaba dispuesta a una pelea de esgrima verbal contra él esta noche. ¡Demonios! Las perdía incluso cuando mi cerebro no sentía que estaba a punto de latir fuera de mi cráneo.
—Detente —le dije, esperando de veras poder lograr transportarme de nuevo antes de perder el conocimiento.

Explanation:
De que se puede decir de varias maneras, claro está, pero ésta es la idea. The person's head was throbbing so much that it felt as if it were going to break through her skull.

Other options might be "(aun/incluso cuando) no tenía el cerebro a punto de salir del cráneo (a golpe de palpitaciones)" o "(incluso cuando) no tenía el cerebreo a punto de estallarme el cráneo (de tanto palpitar/vibrar)"
