English term or phrase: didn't feel like it was about to throb out
Mircea was striding towards us, and I was not up to a verbal fencing match with him tonight. Hell, I lost those even when my brain didn't feel like it was about to throb out of my skull. "Hold on" I said, rally hoping I could manage one more shift before I passed out.

The person's head was throbbing so much that it felt as if it were going to break through her skull.

Other options might be "(aun/incluso cuando) no tenía el cerebro a punto de salir del cráneo (a golpe de palpitaciones)" o "(incluso cuando) no tenía el cerebreo a punto de estallarme el cráneo (de tanto palpitar/vibrar)"

Espero que le sirva. :-)
