Either of these 2 would be OK. Just remember that if you're asking an adult or elderly person the question, you should use "tiene" (as a form of respect). If it's someone your own age, "tienes" is fine.

¿Cuántos años tienes ?


Explanation:
This is the most common way. it means "How many years do you have"
You can also say
¿Qué edad tienes ?, that allows any response, even a few maonths (but babies don't speak) :-))
