Usually, an investigation has to be conducted after an "incidente". This is normal practice in many companies (i.e, General Electric, where I currently work)

Concuerdo con Andino, "a near miss" es sinónimo de "a close call" la cual se define como escapar de una situación difícil, especialmente tratándose de accidentes en los que la persona salió ilesa "milagrosamente."

Yo lo he traducido en ocasiones como "cuasi accidente" o bien "accidente evitado justo a tiempo".
Otra opción: "conato de accidente".

Es buena la respuesta de Andino, pero trata de un accidente que sí se produce pero sin que nadie se perjudique.

"Near Miss" es un accidente que estuvo a punto de producirse pero no pasó nada.

Although the dictionary translates it as "cuasi accidentes" la verdad es que expresion no es muy comun....

Se trata simplemente de accidentes que casi ocurrieron pero que no llegaron a serlo.
