Turkish translation:Orijinal/İlk talimatların tercümesi/çevirisi
Explanation:

I guess this will work:

Orijinal = original
ilk : first/initial

tercümesi vs. çevirisi is matter of preference. I prefer "tercüme" yet younger people may opt it out for it is more of Arabic origin.

As Korhan suggested in the discussions "İngilizce aslından tercüme edilmiş kullanma talimatları" is the most correct translation for this phrase based on the asker's further explanation.

translation of original instructions
Kullanım talimatlarının çevirisi

Explanation:
I would omit "original" - in the Turkish version, it wouldn't be immediately clear that the "original" refers to the instructions in their language of origin, but might mislead the reader bringing up connotations like genuine or initial.
The literal back translation of my suggestion would be "Translation of the Instructions for Use."

Also, if this is a title or sub-heading, I suggest you capitalize all initials, unless the other way around is the preferred format for all such headings.

agree  O. Korhan KARTIN: Usage of "kullanim" is necessary for this context. I would omit "original" as well.

Dear Lenka, you can use following based on my military-service life where I have come across it for numerous times;
" TEMEL TALIMATLARIN TERCÜMESİ " (in English" I should have read" Translation of Basic Instructions) meaning subject to future revision by Manufacturer.
