Explanation:
Desmond's answer is, in principle, correct. The problematic expression for me is 'irti alustastaan'. It definitely refers to the leg.

This is how I suppose the procedure goes: The doctor raises the patient’s leg off the bed ('irti alustastaan'). The higher the doctor raises the leg, the easier it is for the patient to support it himself. In this case, the doctor apparently needs to raise the right leg less so as to the patient can support it himself, compared with the left leg (= oikea alaraaja nousee kevyemmin kannatukseen).

The text isn't very clear, but it is doctor-to-doctor text, and they know the routines and surely can interpret also each other's minimalist wordings.
