Well, as even my husband, being a Brit, had a civil registry number in Belgium when we lived there, that cannot be the one they are referring to. They may just be referring to the personal insurance number given to the patient or insured person by the insurance company, in which case it is entirely possible for a housewife or a child not to have one, and then husband or father/mother needs to provide one. Then the first question refers to merely the order the Civil Registry would use to put down the family (husband, wife, children - eldest to youngest).

It is for the E112 form so this is the only context. The thing that made me wonder about the id number is that following that point, it says in a separate point "if the family member does not have a personal identification number, provide the insured party's number" However this isn't necessarily related as these are separately numbered points at the end of the document.

Ok, so after a computer crash... The 'rijksregisternummer' which would be the number that comes into existence when a civil servant of the service 'Bevolking' (service that takes care of everything apart from divorce, marriage and birth) puts in certain details about a person for the first time (at birth) would be the number you are looking for. It is also the one found on the social security card (SIS) and as far as I know the only number by which a person is really identified in Belgium. However, let us say that a woman is older than her husband, she would have the lowest number as those numbers start with date of birth (year-month-day). so essentially she would come first above her husband in the list of the civil register, but her husband is still, officially, the head of the family. If she does not work, she falls under her husband's social security, as well as the children, although his number may be of a higher order than hers. What is the form for?

thanks kirsten, that is what I thought - I was just clutching at straws! I think it may be in the order of your personal identification number. Do you know if this is generated by the burgelijke stand in Belgium?

Normally the 'burgerlijke stand' is the civil registry, so the place where you declare divorce, marriage or acquisition children. Your civil status (i.e. married or not) is your 'burgerlijke STAAT', so I don't think your translation should read 'in order of civil status'. I would opt for the second possibility you list, but it is not unthinkable that the person who wrote the document is not aware of the difference between 'stand' and 'staat' in this case, depending on the seriousness of the document (context needed) and even then it is very well possible that there is a mistake in the form. Otherwise contact the client himself for further explanation.
