(I have also found "avis de décès" translated as an "obituary" so either this is a translation mistake or the expression has 2 meanings). The "bulletin de décès" is issued by funeral homes. Try typing each expression into Google with +Québec and you'll find quite a lot of sites, some of them more useful than others.
