I don't think there is an exact equivalent for this kind of "self-certificate" in the UK but, if there were, I'd call it that!

I'm not really sure but I would imagine it is a form or slip signed for security reasons before visiting or entering a company.

Explanation:
Oddly enough, we had 'accueil' not all that long ago, just at a time when I had encountered it in a major project I was working on, where there was enough context to really get to grips with the meaning of the term. On that occasion, I arrived at the conclusion that this kind of 'accueil' most closely equates to the use of the term 'induction' similar HR etc. contexts — a short introductory training session giving enough information to a new employee etc. for them to at least enter the site and begin finding their way around.

From the content listed, I'd say it is more geared towards 'safety' than 'sécurité' (which in any case, if a distinction has to be made between the two, is often referred to as 'surété'); I suppose you could if you wished refer to it as 'safety and security'.

As for 'fiche', well, you can choose what you like for that; it seems to me this is really just a disclaimer for the employers to get themselves off the hook, so they can say "Well, they had been told about X hazard"; as such, I think 'form' is probably better, since 'sheet' to me in some way suggests that there might be lots of details to fill in on a sort of questionnaire, which doesn't appear to be the case here.

Well, from the context you've posted, it seems as if it just an acknowledgement on the part of the inductee that they have received basic training / awareness-raising on a list of specifc points. I don't think it looks to me like self-certification, so much as simply a 'receipt for training'
