French term or phrase: tel que vu

I realize this means "as seen". Yet it appears on the bottom of a death certificate, with what appear to be initials.
Could someone please shed some light on what this means, or is some other translation perhaps more appropriate? Thank you.

tel que vu is exactly that: as seen.
When copying an entry in a register, even if there is an error, you copy exactly what is written (seen). But since we are not sure what the document really is and where the "tel que vu" appears, and Asker seems to have disappeared, we can only guess. Which as AllegroTrans says, is a waste of time. And in the end, who cares?

it's no use guessing here, we need proper context from the asker
Otherwise it's a waste of everyone's time
This is a formal legal document and it has to be translated properly

Tel que vu alone sounds to me like "as seen" in usual french. I am not a scholar like you in the field. Can you use it in "tel que vu" instead of "après vérification du registre" or "conforme aux registres"? Can you build the sentence? Thanks

those of the person who provided the information, or merely of someone else who has witnessed its documentation in the certificate?
If the former, then simply "Informant:"; otherwise Timothy Rake's suggestion sounds good.

These expressions are really dependent on context. It would be the surrounding context that determines if they are formal expressions or not. It all comes down to usage of language.
