I am translating a letter in relation to Research Ethics Committee approval on risk evaluation of ICDs.

Here is the context:

Vous voudrez bien nous retourner un original et une copie - XXXXXXXXXXXXXXXX - du formulaire de consentement modifié, dont la copie indiquera en surligné au crayon jaune les modifications demandées, et non pas en mode suivi des modifications. L'original vous sera retourné avec l'estampille d'approbation.
