French term or phrase: Anomalie bloquante
• Anomalie bloquante sur une fonctionnalité stratégique du Système, pour laquelle aucune solution de contournement ne peut être mise en place.

I've seen this so many times but I like to get a definitive way to translate it.

This seems the "zippiest" option, while remaining fairly open (i.e. problem may not necessarily be a bug)

blocking anomaly/problem/trouble/difficulty

Explanation:
Blocking is clear. It prevents progress or processing.

Anomalie is more ambiguous and there may be several options depending upon what the context allows or supports. Basically this is something that didn't quite go right. Any of my suggestions seem to me to be good possibilities but this is a situation where the context rules.

I like irregularity (but it sounds more British than American). I think that "fatal error" and "fault" are too explicit and pejorative about what is the preventing operation of the strategic functionality. Anomaly is vague, which is why it's hard to translate, and why a comparably vague adjective is needed in English.

Fatal error causing the failure of a key operating function within the system

Explanation:
Perhaps long-winded way of saying things, but the IT stuff is never short and sweet! Key=strategic.
