Explanation: To me, "appréciation" implies some form of assessment of a pupil's ability to learn, attitude, etc., not simply "qu'un élève a bien suivi un certain parcours", though the term may reflect a certain "langue de bois éducationnelle".
In English I think you can dispense with "report" unless it is not otherwise obvious that this is the paper which the assessment is recorded rather than the act of appraising itself.
However, if it is indeed more a mark of attendance, then you could use a "bastardized" expression like "assessment/appraisal certificate".
Moving on from there, you could even use "certificate of competence" which, these days at least, does not mean one is at all competent, but that one has sat (or slept) through course.
