Following my previous question, this appears on the first page of the document, just below the heading "Contrat d'assurance sur la vie en devises et à capital variable". I've been looking at the different uses of "souscription" and am a bit wary of this word; here it would appear to be an application form (in fact, a form in which to insert all the details of the insured follows) but I'd like to be sure if "application form" is ok in the context.
