I know what this thing is but I just don't quite know how to put it in English. I have gone with "certificate of authenticity" unless someone can give me something better:

Context:

Document indiquant les caractéristiques mécaniques et l’analyse chimique du matériau utilisé dans une fourniture. Ce document est délivré par le Service Contrôle Qualité interne du fabriquant du matériau ou par un laboratoire extérieur. Le certificat matière fait partie du passeport qualité de l’équipement ou du matériel qui sera élaboré à partir du matériau.

Explanation:
The Materials Audit Unit is responsible for verifying that construction material used in Department Projects is from approved sources and that it has been sampled and tested at the required frequency. Once it is determined that the materials were properly sampled and tested and that they met all Department Specifications, a Materials Certificate is issued
