Le diagnositc retenu est donc celui d'un spasme coronarien survenu au décours de la plongée sur un terrain de cardiomyopathie ischémique ancienne avec séquelle postéro latérale non diagnostiqué.

In this case, does sur un terrain de mean that the spasm stems from previously undiagnosed ischemic cardiomyopathy?

Coronary spam arising with a history of previous ischemic cardiomyopathy that had previously undiagnosed posterolateral residual damage.
The ischemic cardiomyopathy was NOT previously undiagnosed but the subsequent posterolateral damage was not diagonosed.

It doesn't mean that the spasm was directly related to the past episode of myocardial ischemia, but that he had an unknown underlying condition.

In fact it's an old posterolateral myocardial infarction.
