this is on the anesthesia report, it's preceeded by the patient's medical history: (that's after no smoker, and occasional drinker info)

from what I can see, it looks like coumedian
and fentamy (or whatever) is the name of the medication
- although there is nothing in his list of "usual medication" that looks remotely like that - so it must be something that was administered at the Hospital

can anyone decode that?

So I suppose it's fentanyl citrate sold as Fentanyl but Fentanyl panpharma IV is prescribed only in hospitals.

agree  Tony M: Fair enough, you're the expert! It IS available in oral form, but it looks as though 50mg would be lethal!! Perhaps it was 50µg? I just thought it odd the patient should give this in their history as IV, but quite take your points...
