Here's the sentence: Par contre elle trouve très difficile d’accéder aux clubs d’observance

Would you say HIV monitoring units? Or is there a more specific term? Perhaps clubs do exist?

A group approach to adherence is
common in the NGO sector in Burkina Faso, often referred to as “clubs
d’observance” (adherence clubs). In Mali, where the other half of the ATARAO
project is carried out, patients receive group adherence counselling prior to treatment
initiation at certain centres (M.Sicotte, personal communication, July 21, 2008).

En sortant du centre, les anciens pensionnaires participent aux activités de prise en charge communautaire de l’association – “clubs d’observance”, entretiens individualisés, éducation thérapeutique -- pour maintenir ou renforcer le suivi auprès des bénéficiaires. Les anciens pensionnaires reçoivent des vivres, condition importante pour la poursuite des traitements.


When they leave the centre, former residents participate in the association’s community care activities (“compliance clubs,” individual meetings, therapeutic patient education, etc.) to continue or strengthen follow-up care for residents. Former residents also receive meals, which is a critical component of the continuation of their treatments.

agree  DouglasCarnall: yes, I'm not so keen on compliance which has many authoritarian overtones. Adherence is nicely neutral, and probably more appropriate for a patient group/club.

To cope with the ever increasing number of patients and with the goal of reaching 15,000 people by 2010, MSF focused on delivering HIV services in new decentralized health centers using a nurse-based model of care, improving clinic organization and triage of patients, re-defining staff roles and training. MSF also launched new strategies to address the challenges of long-term adherence to ART, including creating ‘adherence clubs’ for stable patients on ART for at least 18 months with no complications.

Adherence Versus Compliance

The terms "adherence" and "compliance" are often used interchangeably. "Compliance" had traditionally been used to describe a patient's ability to take medications as prescribed, but recently this term has fallen into disfavor. Some have suggested that compliance connotes a paternalistic relationship between the physician and patient and that the noncompliant patient performs deviant behavior or exhibits weakness of character. "Adherence" better represents the more complex web among patient, provider, and medication and reflects the fact that following a medication regimen is not necessarily a simple choice.
