Without more context - AT stands for Accident du Travail (work accident). Would make sense in a medical history - what does the file say? Is the patient handicapped?

Sorry, Meant to say that if the term A.T. on your text goes together with "Observations", it's very likely that they mean work accident. Otherwise, if you have more context that'd be great.
