There is no specific medical device - it is a template to be filled in which covers all types of devices, so I presume that, whoever feel in the form, enters the name or reference number of a particular item, and subsequently answers the questions relative to the item

As it is a model template to be filled in, I don't really have much detail.
The template is composed of a set of questions:

point de couleur ou gorge de couleur
détachement du point de couleur, allergie du patient , perte d'indication pour le chirurgien
vérification biocompatibilité du marquage couleur
qualification du procédé.

Please, give us a block of text to look at. What sort of implant? Is the "détachement du point de couleur" a defect? Maybe it has a coloured bit on it that dislodges?
