I also agree with Michael's explanation but the context is different.
There is also the interesting case of NOM;
Non operative management in the case of blunt liver trauma for example.

During elective surgery, seems to kind of sum up the idea best.

mortality during surgery (see explanation)

Explanation:
In fr.answers.com it says this

dit des interventions pratiquées en dehors de poussées inflammatoires

so it seems to me it's talking about surgery undertaken where there is no swelling. Unfortunately I haven't found the correct term (yet) but it seems definitely NOT cold surgery (or due to cold).

Mortality rate on medically stabilized patients

Explanation:
Alain Pommet's answer is exactly on target.

It means when surgery is done on stabilized patient, free of acute complicating risk factors such as active inflammation or infection,etc.

From Petit Robert: "Opérer à froid : pratiquer une opération chirurgicale quand les phénomènes inflammatoires ont disparu."

correction: delete the word "acute" in my explanation. complicating risks of whatever duration is of course what I meant....

agree  Dr Sue Levy (X): non-acute maybe?//I meant non-acute patients or non-acute surgery - your explanation was fine :-)

agree  Angela Dickson (X): perhaps you could clarify whether, as DrManu claims, 'chirurgie à froid' means 'elective surgery'?//Thanks! I was confused...
5 hrs
  -> "c.à froid"also means non-emergent or elective surgery depending on context. here surgery is not elective but has low mortality when pt is stabilized. in other contexts "elective s."may indeed apply for distinction from emergent surgery.
