health care proxy
Explanation:
The term in French is very specific and with a legal connotation. See reference below.
In English, the current terminology is "health care proxy". It is much more than the non-specific word meaning of a person who has the trust of the patient.
"La personne de confiance est une notion de droit français de la santé. Elle a été créée initialement pour favoriser le consentement des personnes hors d'état de s'exprimer, suivant ainsi l'avis n° 58, 12 juin 1998 du CCNE, le Comité consultatif national d'éthique."

" Another form authorizes a specific type of power of attorney or health care proxy, where someone is appointed by the individual to make decisions on their behalf when they are incapacitated. People may also have a combination of both."

UK legal situation

In the UK, people may appoint a health care proxy under the Mental Capacity Act 2005, but it is referred to as a lasting power of attorney. [8]

"Health Care Proxy
As well as detailing specific future instructions for medical treatment, a Living Will can also appoint a Health Care Proxy. This may be a close friend or member of the family who will make decisions on their loved one's behalf, should the need arise."

I thought there would be another French "legal" expression for "health care proxy"

as trusted friend/trustworth person can = personne de confiance
