Yes - the inflammation prevents the sphincters from opening correctly. For me, that's the only thing that makes sense physiologically. You might still want to check with the authors if possible.
Tricky one - at first glance "bloquer la fermeture" would appear to mean "empêcher la fermeture". To me at least ;-)
