Yes - the inflammation prevents the sphincters from opening correctly. For me, that's the only thing that makes sense physiologically. You might still want to check with the authors if possible.
Tricky one - at first glance "bloquer la fermeture" would appear to mean "empêcher la fermeture". To me at least ;-)

What causes sphincter of Oddi dysfunction?
The cause of sphincter of Oddi dysfunction is unknown. Several theories have been proposed including the presence of microlithiasis (microscopic stones in the bile) and duodenal inflammation.

It dawned on me while cooking dinner that "bloquer la fermeture" means "block in the closed position" - fermeture here being the state of being closed rather than the action of closing.
Does that make sense?

On further reading, I have found that there is debate as to whether the SOO includes the pancreatic duct.

The sphincter of Oddi is a valve comprised of circular muscle that controls the opening and closing of the bile duct within the duodenal papilla (ampulla of Vater). Its resting state is contracted—that is, closed. When neural or humoral signals cause it to relax, bile is released into the duodenum to assist in the digestion of dietary fat. There is a separate but less well-defined sphincter controlling the release of pancreatic juice. Typically, ablation of the sphincter of Oddi by sphincterotomy does not destroy the pancreatic sphincter.

Note that the S of O controls the outflow of both bile and pancreatic juices.
If anything is "bloqué" it would be the "ouverture" rather than the "fermeture" of the S of O.

Is this written by a lay person? There seems to be some confusion. Inflammation in the duodenum can affect the sphincter of Oddi and the ampulla of Vater, which would reduce the flow of bile and pancreatic juices into the intestine - and this would slow down digestion. But I can't see how preventing the closure of the S of O can have this effect.
I think you should check with the client.

Yes it definitely says this. "Ils" refers to the irritated intestine and colon, and the text explores the ways in which they can slow down digestion to eliminate the irritating substances.
