From a patient's medical records. My best guess would be: "Medial (or Internal?) Meniscectomy: mid posterior horn" - but I would really appreciate some input from someone who actually knows.

medial meniscetomy: body to posterior horn
Explanation:
Removal of the medial meniscus. There is no "internal" meniscus.

The meniscus has an anterior and posterior horn. There is no middle "horn" in English. The middle part/zone is called the body.
I think the French is referring to removal of the body and posterior horn, rather than the middle of the posterior horn.
