Well, 'formal CV' should be an appropriate translation. I thought of 'formalized CV' but it sounded a bit strange. After all, there are formal CVs and not formalized CV.

"questionnaire personnel détaillé would also mean 'a questionnaire with personal details' or a formal and complete CV.

"questionnaire de personalité" would mean 'a questionnaire with write up on the personality of the applicant/candidate'.
