en mal-être = in a state of unease (I prefer the word "unease" as it's of the most common way of expressing "mal-être"in English).

réel = real

ressenti = felt/perceived/sensed (I think "perceived" works best here).

"Anxiety" is not wrong ; neither is "malaise." I simply prefer "unease."
