pienso que se debe referir a eso, teniendo en cuenta que es un formulario.
se usa mucho el termino \"productor\" en lugar de \"agente\" o \"representante\", como en \"productor de seguros\" en lugar de \"agente o representante de seguros\" o hasta \"vendedor de seguros\".
\"fiscal\" esta relacionado a impuestos asique yo lo dejaría asi.
