(picture of the) Madonna / Blessed Virgin
Explanation:
Why not go the safest way?
Pending further context (i. e. reference to Cranach/German Marian art implied or not?), you may wish to add „in Mariahilf tradition“ or the like.

an image of Mary Help of Christians

Explanation:
These are certainly the same concepts (compare the Wikipedia pages = same feast day and description of its origin). However, I could not get any satisfying results for this as a title of artworks, so there may be another term that is standard in this context.

The Latin is: "Sancta Maria Auxilium Christianorum", but that didn't help either.

neutral  Michael Wetzel: I was also wondering about this, but it seems strange to give a German name to an international iconographical tradition. Does this translation come up anywhere except for the Cranach in Innsbruck?

neutral  Kphred: I agree with Michael here. This would be the proper format if "Maria Hilf" were originally a German. Otherwise, we would have to write "Weihnachten (Christmas)" thus implying that Christmas were a German institution etc.

-> yes, I agree with you... I was also wondering..I found that "Our Lady of Perpetual Help" and "Our Lady of Soccur" are synonims that describe the iconographical subject http://en.wikipedia.org/wiki/Our_Lady_of_Perpetual_Help

neutral  Michael Wetzel: Also thought about this answer, but it seems to refer to a specific work and not an iconographic tradition. Or do the two unlinked references look reasonably authoritative?

-> Good point. More googling later and no, none really authoritative. In fact, most links (tourism basically) don't translate it.

disagree  Annie and Rolf Reiser: the name 'Mariahilfbild' or 'Mariahilf' must be included
