This is a real word, according to MW, but I've never heard or seen it used. "Auffällig" is an adjective with the meaning
-conspicuous
-obvious
-noticeable, etc.

Perhaps the adjective + whatever noun applies here:
conspicuous /obvious / noticeable + deviation / non-compliance / non-conformity
Perhaps this will get you on the right track

  -> If there's only one accepted term, then you're probably right. I'm not specialized in QM specifically, and have been working with Six Sigma a lot lately, which uses this type of language. Thanks for the correction.

This is what they call it in the quality reports of the multinationals I work for. I think you shouldn´t leave out "recurring" or "repeated".
