Explanation:
It is very difficult to tell on the basis of so little context and the misspelling of your source term. A shot in the dark, nonetheless.

The second sentence (see second link below) refers to the carbon-copy Überweisungsformulare that were so common before the banks found it necessary to save money(!) and prior to our knowing anything about Internet Banking.

This is also interesting. If your text is especially current, you might be well served with something like multi-part form, as int he sentence ... the information is then typically taken to a station elsewhere in the hospital to be entered on a State-provided, often multi-part form.
