Even phase transition is correct, I have to add the second common translation as well.

First note that "Fasenumwandlung" is a typo for "Phasenumwandlung". Even the New German grammar allows some transitions from "Ph" to "F", this is not the case with "Phase" as another term "Fase" exists, which has a total different meaning.
