I am not sure if "leaving certificate" is appropriate here.

Clarification request(s) and response

school-leaving report
Explanation:
The first source is a German-English parallel text [pdf] with a glossary and they translate it with 'report' rather than 'certificate' which seems a good solution.

The other source, from the USA [pdf] makes this comment about Abgangszeugnis:
"Note: An Abgangszeugnis from any type of school denotes unsuccessful performance and failure to graduate"

Hope it helps with the doubts

disagree Cilian O'Tuama: you don't get a "(school-)leaving cert" if you drop out of school early

unenrollment record / transcript record

Explanation:
Though not 100 % sure myself, I would probably write:

unenrollment record -- or transcript record.

agree Deborah Shannon: record is a good alternative to certificate here but I'm not sure about 'transcript' as I haven't seen one of these documents, no idea if they list the (fail?!) grades

-> I am pretty sure that they list the failing grades- a transcript normally lists all academic work - audited courses, workgroups, normal courses, etc.

agree msherms: this might be a good alternative as well ! report doesn't have the "successful" ring of certificate
