Zusammenfassend stellen wir gemaess [] HGB fest, dass wir die Lagebeurteilung [] als vertretbar unter der Voraussetzung ansehen, dass die [] Massnahmen erfolgreich umgesetzt werden, und die Annahmen, die der Business-Planung zugrunde gelegt werden, eingehalten werden koennen.

My version
Summing up, we declare in accordance with[] HGB that we consider the assessment of position by the management as justifiable, in particular the acceptance of the continuance of the corporate activities and the assessment of the future development of the company as expressed in the annual accounts and the assessment report, provided that the measures mentioned in the [] are carried out successfully and the assumptions which are taken as a basis for the business planning can be adhered to.

I think "business plan" is more suitable here and you could delete the "which are" in the phrase "assumptions which are taken as a basis" and just go with "assumptions taken as a basis for/underlying"
