I'm not sure I made myself completely clear, so I'll give here the two lines as they are in the text:

Änderungsspesen: J Restsaldo: _____________
Spesen-Kto Nr.: Spesen Whg: _______

I assume the 'J' stands for 'ja', i.e. there's a fee for making changes to the arrangement, and the account from which the money to pay these fees is taken is the same as the account from which the transfer is made. That would make sense, wouldn't it? In that case, maybe 'Spesen-Konto' should be translated with 'Fees charged to account no....".
