Koennen Versicherungszeiten nicht festgestelt werden und gibt es keinerlie Anhaltspunkte fuer die Zustaendigkiet einer anderen deutschen Krankenkasse, bitter wire in den Feldern 6.1 bis 6.10 Fuellstriche anzubringen und den Vordruck nach Ergaenzung des Feldes 8 ebenfalls unmittelbar der in Feld 4 genannten Stelle zu uebersenden.

Explanation:
I tend to believe that "Füllstriche" are dashes to be inserted where (in your case) no proof of health insurance cover exists (in other words, these dashes serve to fill the gaps in items 6.1 to 6.10 of the form). In English, you'd probably use "N/A" or, alternatively, "no information/evidence/proof".
