but aufgenommen und genehmigt is very reminiscent of the French "lu et approuvé" (read and approved)
Is this from Switzerland perhaps?

Hi - thanks very much for the suggestion. Unfortunately that wouldn't work in the context either. Having discussed the phrase with a German lawyer, I am now tending towards "aufgenommen und genehmigt" (recorded and approved) as this would work in the context.
