meeting minutes form
Explanation:
I don't agree with the two other's interpretation of the word. For me it's clear the text talks about a sheet of paper used to take notes about a conversation or meeting.

And because the guys don't use that form, they lose some of their business...

Danke Cristophe! I settled on 'minutes' because it fit my sentence well. Thank you all for your answers (especially Tom and Uli - you two are always reliable when I'm in a tight spot). Now about the stealing, I agree that some answers come simultaneously. When I find the time to answer sometimes, I find out that somebody has already written the same answer or something similar.

I absolutely do no agree that Uli has stolen any answer from anyone. My God, I wish I could pick her brain sometimes, what with its veritable store of goodies. ;-)

I agree with Tom that the tone in KudoZ (and elsewhere in proz.com) should be kept professional. Truculence (read: bitchiness) should be exercised in a bar or railroad junction. ;-)

range of topics [for discussion] / topic range

Explanation:
I immediately flashed on "story arc" (television term), but that, of course, would not fit. A Gesprächsbogen encompasses a certain number or range of topics, so maybe you could use that. I did find a good number of hits on "topic range" as well, see what works better for you.

scripts, etc.

Explanation:
Enter \'Gesprächsbögen\" in www.google.com and you will find many websites.

In fact, these are actual papers, scripts, if you like, that form the basis of \"Mitarbeitergespräche\" and on the basis of which employees may be assessed (it depends on the exact context and use of your particular translation).

Explanation:
Go to the website below; you can call up a sample Gesprächsbogen.

You could even say, "memo of discussion or session"....

script or outline

Explanation:
Upon further reflection, it was the story arc that misled me. These Gesprächsbögen appear to be indeed scripts or outlines for the talks. So I take back my earlier suggestion and amend it as above.

Check this site for an actual sample:

www.consultant-magazin.de/tools4_6.cfm?ID=tools

And, just to show that Gesprächsbogen does have 2 meanings:

uel deserves better, and so do we all...

Explanation:
With all due respect to the other responders, uel deserves better, and so do we all. Well most of us anyway.

Anonymous, truculent and insulting comments like the ones that have been leveled against uel here are -- in my view at least -- inappropriate on this forum and very unwelcome.

Responders to queries don't always see all the other answers first, before they send in their responses. They may have been spending considerable time working on them separately and in parallel with other responders. uel is very thorough, and I doubt very much that she needs to steal.

Explanation:
This covers both forms before a "Mitarbeitergespräch" and reports written afterwards by superiors - assuming the context is personnel assessment. Hope this helps!
