As far as I understand it, these are the offices to which people can apply for help with covering the costs of specific types of rehabilitation or to arrange specific therapies. I'm not entirely sure, though!

I'm reluctant to use "facility", as I don't think it refers to rehab centres or hospitals, etc. I think these offices provide financial help and advice rather than the actual therapies (but I could be wrong).
