This has come up in a similar context, so I'd like to refer you to the Proz.com glossary (and toot my own horn) at:

Now, Kassenarzt and Kassenärztliche Vereinigung cannot be assembled and translated from its parts, but rather this is a somewhat idiomatic term/name. The Vereinigung is not an association of insurances but rather an association of physicians. A Kassenarzt is translated with panel physician, not insurance doctor. Thus, I suggest "association of panel physicians" (possibly only in parentheses and following the original, German term.)
