Does anyone know what a Gefäßwandinsel is?
This is from an operative report of a cardiac surgery involving varius repairs to the ascending aorta and aortic arch.

Context: Eröffnung von Aorta ascendens und Aortenbogen bis in die Aorta descendens hinein. Die Halsgefäße werden mit einer Gefäßwandinsel ausgeschnitten. Dann wird die Aorta descendens mit dem Homograft End-zu-Seit anastomosiert.

Explanation:
Exposure for aortic valve operations after previous coronary artery bypass grafting may be technically difficult owing to the presence of patent vein grafts on the proximal aorta. A patch or "island" aortotomy technique that allows excellent exposure of the aortic valve is presented here. In select patients this approach may facilitate cardioplegia administration.

Aortic arch aneurysms require the reattachment of the arch vessels, the innominate artery, the left common carotid artery, and the left subclavian artery. To decrease surgery time, these three vessels can be treated as a single vessel by using part of the patient's native aorta to create an island. This island is then connected to the graft.
