I am afraid there is no context other than the fact that this is from a chest x-ray.

heart showing aortic configuration

The link below might be incomplete. I got it from Googling "aortic-configuration heart". I didn't get any results for "aortic-configured-heart" or the more grammatical "aortically-configured-heart".

The following sentence comes from the Google excerpt from the PPT file, but I couldn't actually find it in the slideshow.

"The combination of these 2 effects results in a peculiar boot shaped heart called aortic configuration."
