Help with sentence, please

I'm pretty sure that the following sentence is missing some parts. I hope somebody can help me make sense of it. If not, I'll have to add a note to the client.

Bei {Patient} wurde nach gescheitertem Lyseversuch des thrombotisch verschlossenen femoro-cruralen Bypasses eine Thrombektomie und bei Blutung im distalen Anastomosenbereich unter Lysetherapie zu übernähen.

It's probably missing "durchgeführt" after Thrombektomie, and then I would expect something to the effect that a decision was made to suture....

This was probably written by a junior doctor at the end of his 48-hour non-stop stint, let's just fill in the gaps for him as Trudy does

As Ingeborg and others have remarked, the final decision must be taken by the client, especially as it is not quite clear what had to be "übernähen".
My "high confidence" rating was perhaps a bit rash, given that I would find it difficult to make a decent English sentence out of this. But I was (am) 100% sure that there are parts of the sentence missing in two places.

Thanks to everybody for confirming my hunch. I went with "performed" for durchgeführt, and threw the rest back in the client's lap :-) Last I heard, they couldn't figure it out either and are going to ask their client for clarification.
