agree  Siegfried Armbruster: I agree with your translation, but in atrial fibrillation, there are no P waves // that is why I agree with your translation, the 2n part of my comment is more targeted at the source text.

-> no - the ref above is just to show "conduction ratio" - I think the pt probably has atrial flutter rather than fibrillation/OK - thanks Doc :-)

agree  uyuni: with "fixed conduction ratio". @Siegfried: P-waves in AF may be high in frequency, low in amplitude and thus hard to detect or sometimes hardly visible. But after all, there *are* P-waves in AF. The text might indeed be about atrial flutter, though... ;-)

'Kammer' refers to 'ventricle'.

fixe Überleitung auf die Kammer = fixed AV conduction to ventricles

atrial fibrillation with fixed rapid [ventricular] conduction

Explanation:
This is how I would phrase it. Note that atrial flutter, rather than fibrillation, is typically characterized by a fixed conduction; however, it is also possible in fibrillation.

FYI, 'fixed' as opposed to 'variable'
i.e., atrial flutter can also exist with a rapid but variable ventricular conduction
