This is at the bottom of a German medical bill, under the heading "Kostenübernahmeantrag". This is the context:

"Der o.g. Patient wurde zur stationären Behandlung in unserem Klinikum aufgenommen. Die medizinische Notwendigkeit der Aufnahme ist oben begründet. Falls noch nicht erteilt, bitten wir um Übersendung einer Kostenzusicherung."

The whole document is being sent to the employer of the patient. From context I am guessing that the employer is to assure hospital they will pay for the bill, but I wanted to see what the precise term for this would be in English, or any suggestions for a good translation.
