I am a native Swabian speaker and it seems to me that that the patient got her diagnose in Swabian instead of German! We use 'sch' instead of's'! Then the diagnosis makes sense as well! Sometimes and in some professions such as legal professionals or the medical profession speakers of heavy dialects such as Bavarian, Swabian or Aleman really should force themselves to the standard language German although our former prime minister of Baden-Wurttemberg once said: Mir könnet alles ausser Hochdeutsch!

I'm afraid I don't know in what part of Germany this case originated - all the identity details have been redacted. Possibly, the patient misheard the doctor - pustular seems to for in here...
