I assume it's the symptom that's being described as Streckhaltung, not the therapy? (The latter might be s.th. like traction or cervical collar.)

The hyper-ability for ossification, characteristic for ankylosing spondylitis, might be beneficial when it comes to healing of a fracture in a slightly extended position. When a progressive neurological deficit is present despite optimal immobilization, operative stabilization is indicated.
