This is from a patient history. I'm guessing "FA" stands for "Familienanamnese", as under this heading are the diseases of the parents.
Could "SA" be "Sozialanamnese"? It seems to describe the patient's family circumstances, but not in a specifically medical context.
Under "PA", mention is made of a disease from which the patient suffered some years ago.
